We’re preparing to offer skilled nursing support at home through a licensed Registered Nurse. Planned services include nursing assessments, wound care, medication support, and follow-up visits after a hospital stay or procedure.

Non-medical home care provides practical assistance with everyday life, including companionship, personal care, meals, and household routines.
How often can we receive care?
Care schedules are tailored to your needs. Support is available up to 24 hours a day, seven days a week. Contact us to discuss the schedule you’re considering.
Will we have the same caregiver for each visit?
We prioritize familiar caregivers whenever possible and thoughtfully match each client with someone who fits their needs and personality.
How is care paid for?
Root & Rise Home Care currently accepts private payment. We’ll discuss your needs and explain pricing so you can make an informed decision.
What if we’re unsure which services we need?
